New Assessment Procedures for Home Care Assessments
• Call will be made to NY Independent Assessor (NYIA) to request initial nursing assessment
• NYIA will schedule the Community Health Assessment (CHA) and CA (Clinical Appointment) within 14 days
(These appointments can occur M-F 8:30am-5:00pm and Sat-Sun 10:00am-6:00pm)
• CHA appointment -The NYIA nurse completes their assessment via telehealth or in person.
• CA Appointment- The Independent Practitioner Panel (Independent Physician panel) examines the Applicant via telehealth or in person to determine if they are self-directing and in stale medical condition and completes a Practitioner Order for care
• This Order is handed down to the Local Department of Social Services which will use both the nursing assessment and the Practitioner’s Order to create a plan of care (including number of home care hours). An Outcome Notice will be issued which will have Fair Hearing Right s if a denial of care is issued.
If NYIA approves enrollment into Managed Long-Term Care (MLTC) the Outcome Notice will direct that the Applicant contact an MLTC to enroll. This notice will be good for one year as opposed to the prior 75-day deadline to enroll.
*If Applicant is approved for 12 hours or less/day, the plan of care is finalized.
*If Applicant is approved for more than 12 hours or more/day, there will be an Independent Review Panel review. Within 6 days the panel makes recommendation of whether the plan of care will maintain health and safety at home
